GENE EIDMAN TOOK THE PATH TO … THE FARM

After falling and breaking his femur just a month after having his hip replaced, Gene Eidman found himself on the PATH to recovery with the Presbyterian Manors of Mid-America’s PATH® program.

“We inquired about some other places for therapy, but we knew beforehand that PMMA was where we wanted to go. We’ve always heard good things about the program, and if we could get there that’s where we wanted to be. They had an opening and we got in,” said Gene.

In 2012, PMMA launched a program called PATH® or “Post-Acute-to-Home” to strengthen service to older adults. This program serves patients who have been discharged from a hospital, but still require therapy and special skilled care in order to make the transition from hospital to home. By incorporating the PMMA team of medical professionals to develop individualized programs, PATH helps patients regain functionality and promotes a safe return to their home.

“We were there two weeks. We had a room and my wife was able to stay with me the whole time. The nurses were great, and the physical therapists did an excellent job. Overall, it was a good experience and I’d recommend it to anyone,” said Gene.

Recently, PMMA introduced an enhancement to the PATH program called “I Took the PATH to…” supporting our commitment to patient goals. Through a partnership with Aegis Therapies, patients set unique goals upon admission to PATH. These goals are discussed throughout the PATH stay as a way to motivate and honor progress. Upon discharge, goals are celebrated with a photo, the recovery team and then shared with others taking advantage of PATH services.

Gene took the PATH to get back home to the farm. “My goal was to get back to the farm and I made that. It takes baby steps getting back to where you want to be. I’m not where I want to be yet—I still use a walker and want to get rid of that thing—but with the right care, I’ll get there.”
